Admissions

An individual may be admitted to Faith, Hope & Charity for long-term care when it is determined that the individual is in need of and will benefit from our multi-disciplinary program. The total number of residents at Faith, Hope & Charity shall not exceed the current license capacity.

The Iowa Foundation for Medical Care will determine if the individual can be served in an ICF/MR facility through their certification process. The Department of Human Services case management services must approve the placement before acceptance into the facility. The Faith, Hope & Charity Placement Team will meet to determine whether an applicant can or cannot be appropriately served at this facility. Faith, Hope & Charity shall consider individuals for long-term services who:

· Are between 3 and 18 years of age.
· Are diagnosed with developmental disabilities.
· Are eligible for Title XIX, and/or paying privately for the cost of placement at the current per diem rate.
· Are spending the majority of waking hours out of bed; ambulatory or mobile non-ambulatory.
· Are willing and able to participate in a full day of structured therapeutic activities, and to benefit from said activities.
· Have a legally responsible party who will fulfill parental or guardianship duties.
· Do not pose a serious threat to the safety and welfare of self or others.
· Do not require a greater or lesser degree of care or services than available at the ICF/MR level of care.
· Not needing maternity care.
